summary of conditions as discussed with Dr. Vladana Milisavljevic at UCLA

Here is the update from this morning.  Ian is with a great team.  His condition is well understood, but why he has the myriad of issues and how they are contributing to his primary issue, PPHN is a mystery.  We're trying to reach out to the Neonatology Medical Community for any doctors with similar case experience that may be able to lend some ideas.  Please pass along the following info if you know someone in Neonatology.

The following is a summary of conditions as discussed with Dr. Vladana Milisavljevic at UCLA.  This description was typed out by Mike Abercrombie (father) and will be distributed to other professionals who may have similar, or related, experiences treating patients with Ian's unique combination of challenges.

Ian Abercrombie Condition as of Monday 3/22 at 11:45am


  1. Born Friday 3/19 at 11:10pm
  2. Has PPHN
  3. Large for gestational age
  4. Born pre-term at 35 5/7 weeks
  5. GBS positive mother
  6. HSV positive mother, no lesions at time of delivery
  7. Mother had chronic cough for 3 weeks before contractions and hospitalization
  8. Mother was 21 days in hospital with pre-term labor
  9. Prenatal diagnosis of aorta and pulmonary artery dilation, suspicion of connective tissue disorder (Marfan syndrome)
  10. Severely depressed at birth with low apgar scores
  11. Required intubation and chest compression in delivery room
  12. Appearance of baby after birth suggestive of hydrops with significant ascites
  13. Enlarged liver and spleen
  14. Petechia on skin at birth
  15. On chest x-ray, heart enlarged and lungs white out - Survanta given
  16. Initial CBC with hi NRBC count and low platelets.
  17. Initial BMP 4600
  18. Currently on HFOV, NO, sildenafil (just started) and Hydricortisone, dopamine, dobutumine, milrone drips
  19. IVIG given twice
  20. Numerous transfusions of FFP, cryoprecipite, platelets and PRBC
  21. On ampiciline, gentamicin, meropenem, acyclovir, azithromycin, amphothericin
  22. Cardiology, ID and genetics involved
Currently, O2 sats 98-100, blood pressure 76/40, heart rate 153

So far, there has been a lot of terrific feedback from some very wonderful doctors interested in helping. What we have been able to do is to look at that feedback and see that so far, everything anyone has suggested has already been tried or is already in place. While Mike & I are in no way experts, because we have been so informed by the team, we recognize many things that are being suggested and are able to save his medical team from having to answer these questions again. Thank you for contacting Mike, instead of Ian's medical team directly, as it allows the rest of the team to focus their energies directly on our little Ian.

  1. Dear Shari and Mike,
    I left you a message this morning and understand how overwhelmed you must be however, I wanted to send over a quick note. First wanted to say once again congratulations on your sweet little Ian. Also, so sorry for what you are going through. Having had three children with very serious medical conditions I can get a glimpse of what you are feeling right now. Please know that all though we don't know eachother there is a family out there praying for you all and most importantly praying for wisdom for the Doctors to help find out exactly what is causing Ian's condition. Also, praying that it can be repaired and you will be back home soon with your little guy in your arms.

    I contacted all my connections at CHLA and most of them have already been contacted about your son. So hopefully that means you have a great team who is reaching out to everyone. The most difficult part for you and your husband will be that you are your sons best advocate and although you must feel exhausted both physically and mentally you reaching out to "everyone" will be the best for your little guy. Unfortunately, it seems as if they are all waiting to hear from you? Doctors like to help each other however, they also do not like to step on eacothers toes. Again, only stating my experience in the last 7 years with many different hospitals.

    Sherin Devaskar, M.D., Chief of Neonatology at CHLA.
    She is suppose to be outstanding and from what it sounds like by what you described above she could be of great help if she has not already been contacted.
